Just wondering what your recommendation is for making changes on live sites, when some of the involuntary cache-clearing actions are needed (e.g updating roles, content classes etc), especially for sites which are constantly changing/improving?

Unfortunately, difficult to workaround... except maybe by trying to "neutralize" the expiry.php file during the deployment process. Neutralize means : avoid file change with any possible methods (or avoid expired=1 on cluster mode), save & replace with the previous file for exemple,
