Having just spent the last 6 months or so migrating an ez Publish installation running a suite of websites from a Red Hat Enterprise Linux/Apache 2 environment to a Windows Server 2008 r2/IIS7 environment, I feel like coming up for a breath of air.
If this will be of any help to anyone, here is the content of the web.config file that IIS7 places at the root of eZ Publish to incorporate the basic set of eZ Publish rewrite rules.

Sorry to take so long in answering. At first I wasn't using an accelerator, but now am using WinCache.
IIS differs from Apache in several ways, but mostly it seems more strict in URL formation. Using the basic eZ Publish rewrite rules in Apache, the server would serve a URL request with or without index.php/ included. IIS gives an Error 404 - File not found whenever the index.php/ is included.
I am surprised and pleased with the site's performance under IIS7/WinCache. Page delivery speed seems to be uniform and fairly prompt. See for yourself at:
